The landmark investigation of Heiko and Eva Braak on the staging of AD NFTs was first published in 1991 .This paper described the progressive and predictable sequence of buildup of NFTs with AD disease progression. There are 6 Braak stages of NFT propagation that are characterized by both the location and the density of NFTs.

A system formulated in 1991 by Drs. Braak and Braak (Acta Neuropathologica Berl—82:239-259) for staging the severity of Alzheimer’s disease (AD), based on the premise that AD pathology—specifically neurofibrillary tangles—evolve in stages and locations, beginning in the mesial temporal lobe (stages 1 and 2) and extending to the limbic regions (stages 3 and 4)—at which point dementia manifests itself clinically—ending in the neocortex (stages 5 and 6). The pivotal 1991 study by Heiko Braak and Eva Braak did just that (Braak and Braak, 1991). It showed that tau inclusions form in a stereotypical fashion, allowing classification depending on where the inclusions are found and in what number.

The neuropathological staging model proposed by Braak and Braak (1991) implies that the evolution of neurofibrillary pathology follows a predictable sequence and can be ordered in a regular regional hierarchy. A total of 42 cases of an elderly population sample, which had been prospectively clinically assessed, were examined. Clinical diagnosis was made according to the CAMDEX criteria, and
Braak and colleagues reported in 2006 that Braak staging of AD‐related neurofibrillary lesions can also be carried out on IHC stained conventional 5–7‐µm‐thick sections 6. The immunolabeled AD‐related neurofibrillary lesions vary, however, from those seen when using the silver method, that is, Gallyas. Braak's PD staging suggests that LBD is a prion‐like disease. Most typical PD cases fit with Braak's PD staging, but the scheme fails in some cases. Alzheimer's disease, progressive supranuclear palsy, corticobasal syndrome, multiple system atrophy, frontotemporal lobar degeneration, Creutzfeldt–Jakob disease, cerebrovascular diseases, and essential tremor are common misdiagnoses for Braak H, Braak E: Neuropathological staging of Alzheimer-related Orne J, Janus C, Mariash A, Kuskowski M, Hyman B, Hutton M, Ashe changes.
